Per un soggiorno presso «Rückenwind plus» ti chiediamo le seguenti informazioni:

  • Referto medico con diagnosi
  • Elenco aggiornato dei farmaci
  • Copia della tessera dell’assicurazione malattia
  • Pianificazione dell’assistenza, ad es. di Spitex (se disponibile)
  • Disposizioni anticipate del paziente (se disponibili)

L’iscrizione è possibile telefonicamente al +41 (0)56 265 01 76 oppure tramite il modulo qui sotto.

Per i ricoveri d’emergenza, si prega di annunciarsi sempre telefonicamente.
